IOS XR is the operating system run on a number of Cisco's large, service-provider focused platforms. It was originally developed to solve (or at least mitigate) many of the problems inherent in these systems, such as very high interface and routing scale, large numbers of distributed linecards and forwarding ASICs, and very large configurations.

Platforms: The CRS {1,3,X}, the NCS 6000, and the ASR 9000 all run exclusively IOS XR. The Cisco 12000 series also runs IOS XR as well as some (very old) classic IOS code.
